Epidural Hematoma

A 25 year-old man was struck by a baseball in the right side of his head.  Four hours later, he became lethargic, vomited and developed a left hemiparesis.

Axial CT of the brain: note the large convex area of high signal intensity.  This pattern is one of an epidural hematoma which often occurs as a delayed event following skull fracture and bleeding of the middle meningeal artery.  This is a surgical emergency which requires abrupt evacuation.
