HIV Vasculitis - Brain MRI

A 28 year-old HIV positive man presented with progressive cognitive and acute weakness, worst on the left.

Note the Infarctions on Flair and DWI     Note the Dilated Top of the Right Carotid

(Top Left) Axial MRI T2-weighted scan: Note the area of increased signal in the right posterior limb of the internal capsule. (Top Middle and Right) Axial T2-weighted scans. Note the black flow voids - they are much larger than normal, especially in the right internal carotid/middle cerebral and basilar arteries. (Bottom Left) Diffusion-weighted image. Note the high signal bilaterally in the deep white matter denoting acute infarctions. (Bottom Middle and Right) Axial T1-Gad enhanced scans. Note again that the blood vessel that are clearly dilated, especially the right internal carotid/middle cerebral and basilar arteries.
