A 22 year-old woman had a twenty year history of MS presenting first with optic neuritis. Over the last ten years, he had progressive weakness, numbness and ataxia in the lower extremities.

Show the Gadolinium Enhancing Plaques

Axial MRI T1-weighted images with gadolinium. Note that many of the plaques enhance indicating that they are new and associated with a breakdown in the blood-brain barrier. Such a pattern can mimic metastatic tumors or multiple abscesses.
