MCA Infarction with Secondary Hemorrhagic Transformation - Case 1

A 79-year old man developed an MI followed by the acute onset a left hemiplegia and neglect syndrome.

Outline the Infarction          Show the Hemorrhage            Show the Midline Shift          Show the Trapped Contralateral Temporal Horn

CT Scan of the brain (day 3). Note the large hypodensity involving the entire middle cerebral artery territory as well as the deeper basal ganglia. The fact that the basal ganglia are involved denotes that the infarction involved the proximal middle cerebral artery stem, before the take off of the small perforating vessels which supply the basal ganglia. Also note the increased density in the basal ganglia, denoting hemorrhagic transformation of part of the infarct.
