Multiple Brainstem and Cerebellar Infarctions

A 75 year old woman developed the acute onset of ataxia, diplopia and confusion.

MRI T2-weighed axial images. Note the high signals in numerous areas: bilateral cerebellar, left middle cerebellar peduncle, superior cerebellar, superior pons and midbrain, along with one small focus in the left occipital lobe. MRA demonstrated diffuse intrinsic atherosclerotic disease of both vertebral arteries and the basilar artery.
