Herpes Encephalitis - Case 3

A 32 year old man presented with headache, fever and complex partial seizures.

Show the Temporal Lobe Involvement    Show the Cingulate Gyrus Involvement    Show the Insular Involvement

MRI Flair Axial Images - Note the abnormal signal in both temporal lobes (left image). Also note the involvement of the right insula (middle) and the the cingulate gyrus on both sides (middle and right images). CSF PCR subsequently confirmed the diagnosis of Herpes encephalitis.  Herpes encephalitis has a predilection to involve the temporal lobes. It is essential to recognize this disorder early in order to institute prompt anti-viral therapy.
