MCA Branch Infarction

A 55-year old woman with atrial fibrillation presented with the acute onset of an expressive aphasia.

Outline the Infarction

MRI Flair axial scan. Note the high signal in the distribution of one MCA branch involving the left inferior frontal area. This resulted in an expressive aphasia. The precentral gyrus is just posterior to this area and was not involved, hence, the patient did not have any motor weakness.
