Welcome to the Pediatric Neurology rotation at Rainbow Babies and Children’s Hospital. What follows is some helpful information to guide you through your rotation. The Pediatric Neurology rotation is a combined in-patient and out-patient rotation. The in-patient rotation is entirely based at Rainbow Babies and Children’s Hospital. The in-patient pediatric services are broken down into four teams. Neurology is part of the Green team. This team includes Neurology, Cardiology and GI. The in-patients consist of patients primarily on the Neurology team. These patients are also followed by the pediatric residents on the Green team. The consults come from the various teams, the emergency room and the two intensive care units; PICU and NICU (Neonatal intensive care unit)
 
Pediatric Neurology, The Rotation
  
The Pediatric Neurology team: (in-patient)
The team members vary from month to month. The Pediatric Neurology Attending rotates every two weeks so you will probably work with at least two of the attendings during your rotation. The team members will include all or none of the following: Neuroscience medical students, pediatric medical students doing a week rotation, adult neurology residents, pediatric residents, family practice residents, psychiatry residents.
 
The Pediatric Neurology team (out-patient)
 
The out-patient clinics are satellite clinics scattered around the Cleveland suburbs. These include the two west side clinics at Strongsville and Westlake; and the east side clinic at the Landerbrook office. We also have clinics based at University Hospitals. This includes the Izant center which is room 1200 near the atrium right near the big “ping pong ball sculpture. We also have clinics at the Bolwell building. This includes Dr. Green’s multidisciplinary clinics on the fifth floor and Dr. Gold’s neuro-oncology clinics at the Ireland Cancer Center.

The Place

In-patient pediatric neurology: Rainbow Babies and Children’s Hospital (RBC). RBC is divided into “new rainbow” and “old rainbow” “New Rainbow” is the new tower right next to the circle drive on Adelbert. The “old rainbow” is the building around the corner from the atrium. The elevators to the old rainbow are referred as the “Charlie Brown” elevators (when you take them you will know why). These elevators take you to the intensive care units and the diagnostic center, room 669.
 
Neurology patients are typically admitted to RBC 5. If there is overflow, they may also be found on RBC 6 and sometimes RBC 3. Each floor of RBC has a work room where the teams typically meet for rounds.

* Make sure you get your ID coded at protective services before starting the rotation. Security has really cracked down since 9/11 and you need your ID to swipe into every floor on Rainbow!
 
Out-patient Pediatric Neurology Clinics
 
The out-patient clinics are at suburban satellite locations. Maps/directions are included in your packet. There are also some clinics at the main campus. Check the schedule for the month. At the beginning of the rotation, you will be given a schedule/sign-up sheet for clinics. We have various rotators attending clinics including adult neurology residents, family practice residents, child psychiatry fellows and medical students from other services. The maximum number of participants per clinic is one resident and one medical student. Dr. Klein’s and Dr. Wiznitzer’s clinics are only allowed one person per clinic. The sign-up sheets have the name of the attending for the clinic, location of clinic, time (am/pm) of clinic and blank lines for sign-up to attend that clinic. You can contact Cheryl Key, our lead Administrative Assistant for the clinic sign up sheets. Her number is 4-3753
 
You should check with your attending on service as to when he/she will be rounding on the in-patient team. In-patient rounds are mandatory. You can attend clinics in the afternoon. Please try and attend two clinics a week. Because of time constraints and scheduling you may not be able to go in first on your own with the clinic patients. The clinics are still a great chance for one on one teaching and an opportunity for exposure to the various peds neuro attendings. Take advantage of them!!!!!

1) From the North
Take I-71S to Route 82W (Royalton road Strongsville exit). This is AFTER the 42 Strongsville exit! Stay in the left lane toward Howe Road (not the mall lanes) Proceed to #3

2) From the South
Take I-71N to Route 82 Strongsville exit. Turn left on Route 82. Proceed to #3

3) Turn left at Howe Road (the first traffic light. Take Howe road to Drake Road, turn right. Take Drake road to Pearl road. (note: the Pearl Rd. street sign is above the traffic light), turn left onto Pearl road.
 
The building is on the left side. There are water fountains in front.

1) Coming from the West (Avon, Sheffield Lake, Lorain) Take I-90 East to Columbia road exit. Turn right onto Columbia. Quickly move to left lane. Turn left at traffic light onto Detroit Road. The medical center will be on your left, approx. 8/10 of a mile. IF you should happen to pass the driveway, turn left onto Clague Road and another drive is on your left.

2) Coming from the East: Take I-90 West to Clague Road exit. Turn left at the traffic light. Entrance to medical center is on your right.

3) Coming form south: Take I-71 North to 480West . Exit at Clague road. Turn right on Clague road. Just past the intersection of Clague and Detroit roads you will see the entrance to the medical center on your left.

1) From the North: Take I-271 south to Cedar/ Brainard exit. Turn left on Brainard to Cedar, then left on Cedar to Landerbrook drive (first traffic light), turn left. Go approximately ¼ mile. You will pass the post office on your left. The health center entrance is just past the post office on the right.

2) From the South: Take I-271 North to Cedar-Brainard exit. At the exit ramp, follow the signs that say “Brainard/ Cedar EAST”. At the top of the ramp, turn right on Brainard to Cedar (EAST). Turn left on Cedar to Landerbrook drive (first traffic light), turn left. Go approximately ¼ mile to the health center.
 
What is expected of you (summary)
 
On the first day of your rotation please page the attending service pager (31441) to see which attending is on service and what time/ where you should meet them. You can walk over to Mather dorm (continue on down the hall from Hanna House, the opposite way from the hospitals.) Take the elevator to the third floor, turn right and walk down the hall. Go through the door at the end of the hall. Chery Key’s office is here and she can give you the clinic sign up sheet. She will also give you the medical student with number: 36151. PLEASE REMEMBER TO RETURN THE BEEPER TO CHERYL AT THE END OF YOUR ROTATION!!!!!!!!!
 
You will be expected to see new consults on your own to present later to the attending. There are special consult sheets for you to write up your history and physical. Any active consults should have a SOAP note the following day. If you are going to be at clinic, you can sign out the patient to the other members on the team. The patients on the neurology team are followed by the pediatric residents on the orange team. You do not need to write a daily note on these patients. They will be discussed on pediatric rounds by the pediatric resident following them.
 
You should choose one patient a week to formally write up and hand in to your attending to critique your history and physical.
 
Conferences
 
Conferences specific to pediatric neurology include those described below. We would recommend you also attend conferences required by the adult neurology rotation as well if you are on campus and not at a satellite clinic.
 
Pediatric neurology conferences:
 
Pediatric Neurology Clinical Case Conference
This conference starts at 11:30, each Monday. It is ok to bring your lunch. Located in the Lerner conference room second floor. (across from that mobile of hanging shapes) Attendance is mandatory. An interesting case presentation by one of the neurology attendings. Occasionally a patient will attend the conference to be discussed and examined in front of the group.
 
Pediatric Management Conference
Tuesdays at noon, in the Rainbow Amphitheater. Attendance is optional unless there is a pediatric neurology topic discussed.
 
Pediatric Grand Rounds

Thursdays at 8 am. Attendance is optional unless a pediatric neurology topic is discussed.
There are also additional conferences you may attend including neonatal conference at noon on Thursdays and epilepsy conference. You can ask your attending for more info on these conferences if you are interested.
